In fact, preventative and emergency care are completely covered, while other services are provided with discounts of up to 30%!
Membership Dues
- Adult Membership (ages 15 and up)– $33/month ($342/year)
- Child Membership (ages 14 and younger) – $25/month ($290/year)
- Periodontal Plan (Additional) – $45/month ($504/year)
- Processing Fee – $35

Please remember that this savings plan is not dental insurance, and cannot be combined with any other dental plan, workman’s compensation plan, or offer. It is only valid at Family Smiles Dental. Membership fees are non-refundable, and are subject to change annually. You may not substitute one family member for another.
